There is no specific age when a man starts to feel his virility wane. There are three aspects involved: physical strength, courage and sexual potency. But what is the definition of sexual potency? The doctor in me pays a lot of attention to the presence and quality of morning erections. Why? Because these are spontaneous erections with no stimulation, and for a doctor they mean that there is a sufficient secretion of the hormone that is essential for an erection: testosterone. A lack of morning erections means there is some kind of dysfunction. From age sixty, these erections are less and less frequent, which makes a testosterone-based treatment the logical choice.
A floundering libido!
A drop in testosterone goes hand in hand with a lower sex drive and so the libido crashes. An absence of desire, of erotic vision, is characteristic of a brain that is not suff iciently irrigated or impregnated with testosterone, which is the hormone of sexual desire and eroticism. This testosterone deficiency can be felt from age thirty, though it is generally towards age forty that men tend to need to see a specialist, who will prescribe a male hormone assessment that includes the following:
- FSH levels: a hormone in the brain that stimulates the testicles to produce sperm.
- LH: another brain hormone, which stimulates the testicles to produce testosterone.
- We then look at the levels of testosterone secreted by the testicles, especially free testosterone, as this is what enters the cell and is transformed into dihydrotestosterone (DHT), the tri-active form of the hormone.
- The levels of this free testosterone and DHT are measured, and we also measure the protein that transports testoste-rone in the blood, Sex Hormone Binding Globuline (S.H.B.G.). When the levels of this protein secreted by the liver become too high, it can neutralise the hormone which is then “stuck” to its surface. This is sometimes characteristic of sexual aging and a loss of sexual potency.
- Finally, we measure the presence of oestradiol, which is the feminine fraction of our male hormones. If the oestra-diol levels are too high – whatever their age – we need to suppress this hormone, most often using a hormonal dihy-drotestosterone gel or using other therapeutic substances.
Erection medicines
Sometimes these are required: inhibiters of the enzyme phosphodiesterase with names such as Sildenafil, Tadalafil, Vardenafil, etc. I usually prescribe low doses of 5mg/day and, more importantly, 5 to 10mg an hour before sex to boost sexual potency. The power of penetration and maintaining an erection are fundamental. Another non negligible aspect is premature ejaculation, an expression of the weakening of this potency. A man can be very powerful and sexually domi-nant but can quickly crumble due to premature ejaculation. Note that intercourse should last an average of 7 minutes. And when we talk about premature ejaculation, we mean ejaculation that happens either immediately after pene-tration or within the following minute.
Hormone replacement therapy in men is unfortunately not as widespread in the medical industry.
